Spinning the Web: Integrating Services to Provide Person-Centred Care
Navigating the system to find the right services to meet their care needs is a complex and difficult task for many older people and their carers. Across all sectors, providers are strengthening ties to spin webs of integrated care, improve access and allow for tailoring of services to meet individual care needs.
Services in different sectors are committed to “consumer-directed care”, “patient engagement” or “patient empowerment”, with no consensus on models or definitions of self-determination in care decisions. This seminar will explore how health and aged care services can work together to place the older person at the centre of their care.
