On The Road Again: Optimising Older Persons Recovery Following a Hospital Admission
Many western countries are engaged in an iterative redesign process as they attempt to minimise hospital length of stay and improve the integration of health and social services. What are the issues facing older people who have experienced an episode of hospitalisation? How do community care providers optimise recovery for these people?
This event will discuss the impact of hospitalisation on the older person and provide practical information about ways in which clients and care providers can continue the client’s restorative process. There will be an opportunity for discussion at the end of the presentations.
