Advanced Care Planning: It's about Respecting Choice

Advance Care Planning (ACP) is the process of planning for a person’s future health and personal care. ACP helps ensure that individuals’ choices are respected for future medical treatment. Their beliefs, values and preferences are made known, in order to guide future care in the event that the person is unable to make decisions or communicate.

The forum will cover the whole of the ACP process with a focus on communication, ‘difficult conversations’, when to enact a plan, quality, validity, and compliance of completed ACPs. It explores the challenges faced by health services to ensure an individual’s plan is visible across a diverse range of health providers.
